About the demo form

Our demo form is an example of a very simple contact form. You can of course create far more complex forms than this with Web-form-buddy, and it's easy to bolt-on extra functionality - such as secure SSL mode, file uploads, payment forms (including integration with PayPal and Google Checkout), CAPTCHA tests (to stop the bad guys from abusing your forms), and much more!

The look and feel is entirely up to you!

The way this page looks is not dictated by Web-form-buddy (unlike all other "remotely hosted" form solutions). In the case of this particular web page, the colors and style of the page are set by the little CSS section in the <head> area of the HTML code. But you can design a page like this in any way you choose using your favorite web authoring software, and it will work just fine. All you need do is point the form to your Web-form-buddy script. And what does that mean? Well take a look at the form tag in the HTML source code of this demo web page. It looks like this:

What features are enabled for this demo form?

To give a taste of what Web-form-buddy can do, we've enabled a few popular features for this form. You can modify these and add additional features from your Web-form-buddy control panel.

  • Several of the fields are "required" fields. Try hitting the submit button without entering your name. Notice what happens: the problem field is highlighted and and a "help" message is displayed. If you design large forms that scroll down the page, Web-form-buddy automatically scrolls to the problem field in order to make life as easy as possible for your web user!
  • The email field is checked for validity. As well as a basic test test to check that the email field "looks" correct, Web-form-buddy also performs a real-time network test to ensure that there is a mail server running to accept delivery to that email address.
  • The form completes with a confirmation screen and CAPTCHA. This is an optional feature that allows your web user to check their data before confirming their form submission. Your web site visitors will thank you for this, but this two-stage process is also highly effective in deterring spammers.   • Auto-responder message. Once this form is completed, Web-form-buddy will send your website visitor an automatic email message on the lines of "Thank you for your form submission. We will reply shortly". The contents of this message are fully configurable from your control panel.
  • "Smart submit" button. This is an optional feature that both looks good and serves to prevent duplicate form submissions. Once the submit button is pressed, the button is disabled and flashes slowly to indicate that the form is being submitted. Without this feature it can happen that someone with a poor Internet connection and a slow computer will push the submit button several times.
  • Real time data. This is enabled with the option to show your website visitor's IP address.

Dental Insurance

Our comfortable office is conveniently located one block from the Ballston Metro in Arlington Virginia. Your dental insurance is welcome. If you want to use your dental policy in our office, we are here to help you!  Most of our patients have dental insurance which helps pay for care in our practice. Our administrative team will file your dental claims and include copies of x-rays and clinical information as necessary to help you obtain the highest insurance benefit possible. We are a private practice and have no contracts with any specific dental plans, however, our fees are fair, affordable, and tend to be less than what dental policies allow for our area.  For preventive care, such as cleanings, exams, and x-rays, we can receive payment directly from your insurance company as long as you provide a major credit card to cover any co-pays.  At your first visit, we will sit down with you to review how your dental policy can help you. We are here to make this aspect of your care as easy and painless as possible.

Why should I get my teeth whitened?

Is tooth whitening safe?

Yes, tooth whitening is safe and effective. Using FDA approved tooth whitening gels, you can brighten your smile six to ten shades in a short amount of time. The most common side effect from tooth whitening is having sensitive or irritated gums for a short amount of time. This can be corrected by having a fluoride treatment. Dr. Kim can address any concerns you may have about tooth whitening during your initial consultation.

How does tooth whitening work?

Depending on the results that you want, Dr. Kim can prescribe any number of different treatments to give you a beautifully white smile. Porcelain veneers, dental bonding, take-home kits, and Zoom are all different treatments Dr. Kim uses to give you the teeth whitening results you want. Unlike over the counter treatments that can take months to give you results, Dr. Kim will prescribe you a tooth whitening treatment that will dramatically whiten your teeth in as little as one hour. Special tooth whitening gel that contains higher amounts of hydrogen peroxide or carbamide is applied to your teeth. If you are using an at-home kit, you wear trays filled with gel to sleep. If you are having an in-office tooth whitening treatment like Zoom, the gel is applied to your teeth while a special light activates it. The gel is designed to penetrate the enamel of your teeth and bleach stains that have been caused by age, smoking, foods, or drinks. Zoom works by using the special light to activate the gel, and remove stains faster.

Full Mouth Reconstruction

Full mouth Reconstruction may be required for trauma, injury or severely worn teeth. In addition, when gum disease is severe, with extensive bone loss, it is not always possible to retain the natural teeth and Full Mouth Reconstruction may be recommended. Dr. Kim excels with these cases where patients need Full Mouth Reconstruction to rebuild and restore all of their teeth.  This treatment may incorporate multiple specialists and a vast array of restorative procedures, often including dental implants.

Teeth Whitening

Our practice offers two choices for whitening your teeth. One option is a 90-minute procedure called Zoom. The other is a take-home system called Night White. Both of these options are equally effective, but they vary in cost and time. Prior to whitening your teeth, you will need a dental exam with x-rays, and any active decay will need to be removed and the teeth protected.   • Replacing All of Your Teeth If you are missing all of your teeth, an implant-supported full bridge or full denture can replace them. Dental implants will replace both your lost natural teeth and some of the roots.
  • Sinus Augmentation A key to implant success is the quantity and quality of the bone where the implant is to be placed. The upper back jaw has traditionally been one of the most difficult areas to successfully place dental implants due to insufficient bone quantity and quality and the close proximity to the sinus. Sinus augmentation can help correct this problem by raising the sinus floor and developing bone for the placement of dental implants.
  • Ridge Modification Deformities in the upper or lower jaw can leave you with inadequate bone in which to place dental implants. To correct the problem, the gum is lifted away from the ridge to expose the bony defect. The defect is then filled with bone or bone substitute to build up the ridge. Ridge modification has been shown to greatly improve appearance and increase your chances for successful implants that can last for years to come.

What Can I Expect After Treatment?

As you know, your own teeth require conscientious at-home oral care and regular dental visits. Dental implants are like your own teeth and will require the same care. In order to keep your implant clean and plaque-free, brushing and flossing still apply! After treatment, your periodontist will work closely with you and your dentist to develop the best care plan for you. Periodic follow-up visits will be scheduled to monitor your implant, teeth and gums to make sure they are healthy.

Cosmetic Dentistry

When Dr. Kim asked if I would be willing to write a testimony, I said I’d be delighted. As a result of his work, I could easily be the next poster-child to advertise the “Wonders of Modern Dentistry” if I didn’t mind people seeing both the before and after shots of my teeth. Which I do—they were that bad. My previous experiences at the dentist were awful. From my first set of braces at age eleven to several orthodontists later, I thought there was no hope left for me and my smile. Finally, after 14 years of procrastination, I made an appointment with Dr. Kim. By this point, I'd become a master of the close-lipped smile in photos as I was constantly self-conscious every time I laughed. It could include things like: Number Ten: An office suite that feels more like a plush spa, not a sterile cold dentist’s office. Number Nine: An office team and dental assistants, who are so lovely and kind that you will want them for your best friends. Number Eight: State of the Art equipment that will amaze and instruct you. But what I really want to tell you about is what Dr. Kim and his team did for my husband, Ken. Ken lost several teeth in an accident. In addition, he had serious gum disease and bone loss due to years of a damaging medication. His mouth was painful, unhealthy and unattractive. When I first met Dr. Kim, I explained that I had two goals. First, as an active military reservist who might be called to active duty at a moment’s notice, I needed to ensure that my dental health was in top shape. My first visit to Dr. Kim's office happened unexpectedly; while throwing around a baseball with some friends last spring, I was hit, badly fracturing one of my front teeth. Dr. Kim saw me right away and in just minutes did bonding that looked and functioned quite normally. From that point it was clear that he knew what he was doing. Step 1: You can pay for your treatment the day it is recommended and receive a 5% discount. We will file your insurance claims for you as treatment is rendered and the insurance checks will be sent to you at home.


Step 2: You can choose a long-term payment plan through Care Credit, a separate financing company that offers loans to our patients so they can budget for small, monthly payments and still move forward with care immediately.


Step 3: You can pay each visit the day of the treatment before care is rendered.

Karen is our Registered Dental Hygienist.  She has had years of experience in dentistry as an assistant and a hygienist.  She is sweet-natured with a wonderfully gentle technique.  She receives many compliments and has a history of providing compassionate care to our patients.  She is easy to talk to and has a wealth of knowledge. 

Sallie is our Registered Dental Hygienist.  She brings energy and enthusiasm to our practice and has a way of making our patients feel at home from the moment they meet her.  She is constantly learning about new products for home care and can provide great recommendations for over-the-counter rinses and electronic tooth brushes. 

Stephanie is our Practice Administrator. She recently relocated to the D.C. area from Arizona and will be the face you see when you come thru the door. She is excited about the change of environment and is really happy about working with Dr. Kim and his Staff. With almost a decade of experience in the dental field, she is a great resource for information on the best ways to pay for dental care using insurance and pre-tax money. She is determined to go above and beyond to make sure your experience is enjoyable and as comfortable as possible.

Lyndsey is our Patient Coordinator.  With many years of both medical and dental experience, she brings talent and skill to our team.  Lyndsey will meet you at your first visit.  She will be the team member who takes your picture and x-rays and gives you a tour of our beautiful suite.  Lyndsey cares for our patients in the front and back office and is able to help you with clinical and administrative issues.
